Beginning Winter 2019, all players / coaches must complete registration online. No paper forms will be accepted. This process includes the waivers listed below. No player will be allowed to take the field without all required waivers being completed online.

If you need a hard-copy of the waiver. Simply CLICK HERE to download, sign and print a waiver and give to a Sportzone employee prior to the first game. By signing this paper waiver, you are also Acknowledge, fully understand and agree to all Individual waivers below.
